  Job Title:  Neighborhood Coordinator
  
Department:  Long Term Care Center 

Status:  Part Time (24-30 hours / week) or job share to Full Time
  
Shift:  Variable day-time shifts depending on part-time or job share status (hours weekends, and holidays may vary with responsibilities of position)

Description:  The Neighborhood Coordinator is a blended role with an allocated amount of time devoted to the effective coordination of the daily operations of the  Neighborhood, to include activities, housekeeping and foodservice. The primary purpose of this position is to enhance the resident’s quality of life by working closely with the resident, family members, RN and Neighborhood interdisciplinary team. This individual must be committed to an environment that focuses on the resident and exhibits a genuine interest in an appreciation for adults in the later years of life.
  
Requirements: Minimum 5 years experience in long term care and knowledge of Culture Change practices, Certified Nursing Assistant with current Wisconsin Registry (or obtained within 6 months of hire date) or other licensed Healthcare Professional, demonstrated leadership skills, able to work independently, critical thinking and problem
solving skills, excellent interpersonal and communication skills.

Job Title: Physical Therapy Aide 

Department:  Physical Therapy

Status:  Full Time (.9 FTE) / 36 hours a week

Shift Time:  6:00am – 2:00pm Monday – Friday / hours may vary based on department needs / may also include some Saturdays

Description: Primarily responsible for clerical duties to include greeting and scheduling patients, insurance verification, and assisting with posting patient charges in the computer order entry system. Other responsibilites as needed include for provision and administration of specific physical therapy treatments only under the direct supervision of the staff Physical Therapist.  The Physical Therapy Aide as directed and supervised by the staff Physical Therapist, assists with restoration of patient functioning to prevent disability following injury, disease or physical disability.  Assists patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.  The Physical Therapy Aide participates in operational aspects of the department, maintains performance improvement activities within the department and participates in Performance Improvement activities.  Participates in all infection control, departmental equipment training, organizational safety and fire safety.

Requirements:  Current C.N.A. in WI Registry or must obtain within six months of hire date, previous clerical experience, minimum of one year experience as Physical Therapy Aide preferred.

Job Title:  Tenant  Assistant

Department:  Assisted Living

Status:  PRN    

Shift:   8:30am -12:30pm Saturday and Sunday / additional shifts and hours as needed

Description:  The Tenant Assistant is responsible for assisting tenants with activities of daily living (ADL’s), instrumental activities of daily living, and medication administration as identified in the Tenant Service Agreement. Other responsibilities will include laundry, housekeeping, and activities as specified in the service agreement and assigned by the Assisted Living Director.

Requirements:  High school diploma or equivalent, current C.N.A. in Wisconsin Registry, previous experience in Assisted Living preferred. Must be able to work independently and successfully complete the medication administration class.

Department:   Dietary

Status:  Full Time .9 FTE / 36 hours a week

Shift Time:  6:00am – 2:30pm / every other weekend and rotating holidays

Description: Under the direct supervision of the Executive Chef the cook must be self-motivated, responsible and able to assist with the facility’s needs by preparing meals of the highest quality in accordance with the standards set by HAMH & Water’s Edge. Individual will maintain a positive, respectful and caring attitude, have good verbal and written communication skills, and be capable of using good sound judgment.

Requirements:  Previous experience as a cook with working knowledge of proper food storage, preparation and handling, per state and federal regulations. High school diploma or equivalent

Job Title:  Medical Technologist or Medical Lab Technician                   

Department:  Laboratory

Status:  Part Time (.4 FTE)/16 Hours a Week                                                                                              

Shift Rotation:  Variable hours based on department need to include rotating weekends, holidays and call

Description:  Responsible for processing of laboratory specimans and reporting of results in all areas of the clinical laboratory. Maintains laboratory records. Follows laboratory policies and procedures; maintains quality control practices in the Laboratory.

Requirements:  Bachelor of Science degree in Medical Technology, or chemical, physical or biological science or an Associate degree in Laboratory Science or Medical Technology. Certification as a Medical Technologist / Medical Laboratory Technician through the American Social of Clinical Pathologists or equivalent preferred, previous hospital laboratory experience preferred.

Job Title:  Certified Nursing Assistant              

Department:  Hospital Medical/Surgical

Status:  Full Time .9 (36 hours a week)            

Shift Rotation:  PM (3:00pm-11:30pm) To include every other weekend/holiday rotation.

Shift Length:  8 hours

Requirements:

  • High School graduate
  • Graduate of an accredited Nursing Assistant Program
  • On the Wisconsin Registry of Nursing Assistants
  • Knowledge of medical terminology
  • 18 years of age with one year of work experience
  • Good mental and physical health
  • Ability to meet the typical physical and mental demands of the job
  • Able to meet the essential functions of the job description

Description of Position:  As a CNA you work under the direct supervision of the RN providing care to our patients. You take and record vital signs, provide bath and hygiene care, meal assistance, and assistance with ambulation and transfers. Communication with the patient, family and staff necessary.
